PASSPORT / POLICE CLEARANCE CERTIFICATE/ EMERGENCY CERTIFICATE/ SURRENDER OF INDIAN CITIZENSHIP ETC

1. The Embassy of India, Armenia has been integrated with the Global Passport Seva Project of the Government of India. To apply for Passport, Police Clearance Certificate, Emergency Certificate, Surrender of Indian Citizenship etc., Indian nationals living in Armenia and Georgia are advised to register online and apply for Passport & Passport Services by visiting the link: https://embassy.passportindia.gov.in/

2. No Prior appointment is required for Indian nationals for submission or collection of passport / police clearance certificate/ emergency certificate/ surrender of Indian citizenship etc.

3. All applicants are requested to exercise due care while filling the online application form for relevant service and submit complete documents.

4. The applicants should:

  • Visit government of India link https://embassy.passportindia.gov.in  for application. It is advised that no links other than the links mentioned above should be used to apply for passport.
  • Fill up relevant application form online and submit it providing all the information required.
  • Take the print-out of the filled-in application form.
  • Paste photograph and
  • Put applicant signatures at designated places (minors/ individuals who are unable to sign may put their thumb impression.)
  • Submit in person the (i) filled up and signed application form; (ii) Four (4) colour photographs with white backgrounds (51mm x 51mm); (iii) self-attested copies of current passport; (iv) copy of Temporary/Permanent Residence Card; (v) Student Card (if studying) and (vi) Indian address proof like Aadhar Card, Voter ID Card, Electricity or Water Bill etc. (any one) at the Embassy of India, Yerevan.
  • Applicants should also carry the originals for verification at the embassy.
  • Deposit requisite fees.
  • Ensure that the documents are self-attested before submission at the Embassy. The requirements of additional documents for re-issue of different categories of passport are given below

5. Applicants residing in Georgia may send their application by post, courier or through authorized representative holding notarized power of attorney. Only one application will be accepted from authorized representative.  The Embassy discourages availing/rendering services through agents.

6. There is no Tatkal Service as passports are printed in India.  The average time for delivery of passports is three to four weeks after the receipt of fees and verification in India. The original Passport has to be submitted with the application, because it cannot be used for traveling within the period. The old passport may be resubmitted for cancellation at the time of delivery of new passport. The original Residence Card, after verification, will be returned to the applicant.

7. Application for all types of passport like new passport for newborn child, damaged or lost passport / normal re-issue / exhaustion of pages and passport related services like Emergency Certificate, Police Clearance Certificate and Surrender of Indian Citizenship, is also required to be applied online.

8. The Police Clearance Certificate is issued only after necessary clearances are received from the concerned authorities in India. The applicants are, therefore, advised to apply for PCC sufficiently in advance.

9. Photo Specifications for Passport Services: four (4) colour photographs with white background (51mm x 51 mm).

10. There is no provision for online system for payment of fees it has to be deposited only in the Embassy of India, Yerevan. Fees are accepted in cash and in Armenian Dram only.

11. Once the passport/ document is ready for collection, the applicant will be informed either by e-mail or phone.

12. While collecting the passports/ document, the details should be thoroughly checked by the applicant and in case of any discrepancy, it should be brought to the notice of the Consular Officer immediately.

13. All Indian nationals coming to Armenia and Georgia and intending to stay for extended period are encouraged to register themselves with the Embassy by filling up the Registration Form.

ADDITIONAL DOCUMENTS FOR DIFFERENT PASSPORT RELATED SERVICES

 

SERVICE TYPE

1.

Lost Passport

(i)         Police Report (in original) along with its duly notarized English translation .
(ii)        Annexure-F                 
(iii)   In case of student, bona - fide certificate from concerned University / School may also be submitted.
(iv)   Photocopy of lost passport.

Emergency Certificate

  • Police Report (in original) along with its duly notarized English translation (in case of loss of old passport)
  • Annexure -F (In case Passport is lost or damaged)
  • Emergency Certificate can be issued only after the Indian identity of the applicant has been established.
  • In case of LOSS OF PASSPORT, if the passport is found after the issuance of Emergency Certificate, the same should immediately be reported to the Embassy for advice.
  • It should also be noted that after the issuance of Emergency Certificate, the existing passport does not remain a valid travel document. Hence, only EMERGENCY CERTIFICATE should be used for travel and submitted at the immigration checkpoints.

Addition / deletion of spouse’s name in passport

(I)     Court Decree dissolving the marriage / Death Certificateof spouse, duly attested by Home Department of the State and counter attested/apostilled by Ministry of External Affairs in case of deletion of spouse’s name in the passport.
(ii)    Apostilled Marriage Certificate (with notarized translation in English) in case of addition of spouse’s name in the passport along with self-attested copy of the passport.

Change of Name

(I)     In case of minor changes in the name (like separating name into name & surname), two public documents in the new name should be submitted like school or college certificate.
(ii)    In all other cases, the following documents to be submitted:

(a) Sworn Affidavit for Change in the name (Annexure E )
(b)        Cutting of two leading newspapers (one in the place of permanent residence in India and the other in Armenia or Georgia) carrying advertisement notifying the change of name and asking for any objection thereof to be reported to Embassy of India, Yerevan (Armenia) within 30 days of publication.

Change of Name after Marriage / Divorce or Death of Spouse

(I)  Marriage Certificate / Court Decree dissolving the marriage / Death Certificate, duly attested by Home Department of the State and counter attested/apostilled by Ministry of External Affairs as the case may be.
(ii)  Sworn Affidavit for Change in the name (Annexure E)

7. 

Change of Address

(I)  Valid residence proof like Aadhar Card, Voter ID, Property Papers, running bank statement, Electricity or Water Bill (any one).

8. 

Change in Appearance or Signatures.

  • (Affidavit )to be submitted for Change in Appearance or Signatures.
9. 

Minor Children (below 18 years of age)

    • Original Passport along with self-attested copies of parent’s passport (first & last page).

(ii)    Original Marriage Certificate, duly apostilled along with notarized translated copy in English with self-attested copy.
(iii)  Consent / Declaration from the parents that they have no objection to issue of passport and the child has not acquired the citizenship of a foreign country. (Annexure D ).
(iv)  In case of single parent or consent from one parent is not available, Affidavit is to be submitted by the parent (Annexure C ).
(v)  Self-attested copies of valid Residence Cards of parents.

REGISTRATION OF BIRTH AND ISSUANCE OF PASSPORT FOR NEW BORN CHILD

(a)        Online registration of birth &
(b)       Issue of fresh Passport for an Indian child born in Armenia / Georgia (https://embassy.passportindia.gov.in)
(c)        Name of a child of Indian origin born in Armenia/Georgia cannot be entered on their parent’s passport any more. For a child to be given an Indian passport, his/her birth must have been registered with the Consular Section of the Embassy  http://indiancitizenshiponline.nic.in/.

(d)        Indian Citizenship of a newly born child in Armenia/Georgia:
With effect from 01.07.2012, the Government of India has mandated that birth of a newly born baby outside India for claiming citizenship be registered by descent under Section 4(1) of the Citizenship Act, 1955.The birth of a child is required to be registered within one year of birth.  The Embassy of India in Yerevan will provide the citizenship if the application is made within one year of child's birth. If the application is made after one year, the citizenship will be granted after submission of satisfactory reason(s) for delay in registration of birth of the child beyond one year.

Documents  Required

1. Application for registration of the birth of a minor child to an Indian consulate under Section 4(1) shall be made online and shall be accompanied by an undertaking in writing from the parents of such minor child that he/she does not hold the passport of any other country.

2. For applying online, please visit web site http://indiancitizenshiponline.nic.in/. After filling up the application online, the hard copy, along with the following documents, should be submitted to the Embassy of India in Yerevan (presence of both parents is mandatory):


(I)       Original Passports of both parents along with self-attested copies of parents’ passports (first & last page);

(ii)      Original Marriage Certificate, duly apostilled along with notarized translated copy in English with a self-attested copy;

(iii)     Consent / Declaration from the parents that they have no objection to issue of passport and the child has not acquired the citizenship of a foreign country (Annexure D);

(iv)     In case of single parent or consent from one parent is not available, the Affidavit is to be submitted by the parent (Annexure C).

(v)       A copy of the certificate of Indian citizenship if acquired by registration/naturalization.

(vi)     Birth Certificate apostilled by the local Government along with its English translation, duly notarized along with one photocopy.

(vii)    Self-attested copies of valid Residence Cards of parents.

Additional Documents for Child Born through Surrogacy
1.         In addition to documents required for issue of new passport for a new born child, the following documents must also be submitted: -
(I)       Surrogacy Agreement.
(ii)    Bona fide certificate issued by the Fertility Clinic / Centre where the surrogacy was carried out.
(iii)    Birth Certificate issued by the competent authority in the name of the genetic parents of the surrogate child.
(iv)     DNA profiling test establishing the parentage of the surrogate child issued by Government or Government recognized laboratory.  In case there is no Government recognized laboratory, affidavit [Annexure O ] may be submitted.

ATTESTATION SERVICES
The original document required to be attested may be submitted along with one photocopy and the following documents: -

  • Form 2 (along with 2 photographs)
  • Original Passport along with self-attested photocopy of the first two and last two pages of the passport.

(iii)      Original Valid Residence Card along with self-attested photocopy.
(iv)  The documents originated in India are required to be attested by Branch Secretariats (MEA) or CPV division of MEA, New Delhi. For documents originating in third country, they should first be apostilled by the Embassy/Ministry of Foreign Affairs of the country or the concerned Indian Mission abroad.

INTERNATIONAL DRIVING PERMIT
Indian nationals currently residing in Georgia whose International Driving Permit (IDP) have expired and wish to renew the same, are required to submit the following documents in the Embassy of India, Yerevan:

  • Form No. 2 (Miscellaneous Services) – two photographs, one to be pasted on the form;

(ii)       Passport (in original) along with self-attested photocopy;
(iii)      Temporary/Permanent Residence Card (in original) along with self-attested photocopy;
(iv)      International Driving License (original) along with self-attested photocopy;
(v)       Valid Indian Domestic Driving License (original) along with self-attested photocopy; and
(vi)      Consular Fee of AMD 6,950/-.

The Embassy will issue a verification receipt on the basis of the above documents. The applicant will directly apply for re-issue of IDP through the portal www.parivahan.gov.in and will upload the requisite documents including the verification receipt issued by the Embassy. On receipt of the application through the portal, the licensing authority, after verification of the documents, shall issue the IDP and shall courier it directly to the address of the applicant.
